Sending a Check by Mail
If you would like to donate by mail, you can send a check made payable to Dialysis Clinic, Inc. to the address below. Please indicate on the memo line of your check if you would like to direct your gift to a specific initiative or clinic, or if your gift is in memory of a loved one.

Memorial Donations
If you would like to direct donations to DCI in memory of your loved one in their obituary or memorial information, please use the following language:
“In lieu of flowers, donations to Dialysis Clinic, Inc. in memory of [Honoree’s name] may be sent to Dialysis Clinic, Inc., Attn: Foundation Division, 1633 Church Street, Suite 500, Nashville, TN 37203. Please indicate on the memo line of the check that the gift is in memory of [Honoree’s name]. You can also make an online gift here. DCI is a national not-for-profit organization that provides care along the continuum of kidney disease, and your donation is tax deductible as allowed by law.”
If you would like memorial gifts to be designated for a specific clinic or initiative, please contact the Foundation Division to set up a memorial fund and provide specific language for use in your loved one’s obituary.
Donor-Advised Funds
Donations to DCI can be sent directly from your donor-advised fund to our mailing address (Dialysis Clinic, Inc., Attn: Foundation Division, 1633 Church Street, Suite 500, Nashville, TN 37203).
