What is an ESRD Seamless Care Organization (ESCO)?

An ESRD Seamless Care Organization (ESCO) is a partnership among groups of health care providers and suppliers including Medicare certified dialysis facilities, nephrologists and other Medicare enrolled providers and suppliers located in a contiguous geographical area. Through the ESCO model, these partners are able to work together to enhance patient-centered care, improve communication and coordination of care and access to services. In addition, these partners test a new payment model and care delivery method specifically for Medicare beneficiaries with ESRD.

 

What is the CEC Initiative? 

The Comprehensive End-Stage Renal Disease Care Initiative was announced by the Centers for Medicare and Medicaid Services Innovation Center (CMMI) in February 2013. The CEC Model is a demonstration program that is designed to test, identify and evaluate new ways to improve care for Medicare beneficiaries with end-stage renal disease. The CEC Model aims to improve health care delivery and outcomes while reducing the overall cost of care.

 

Which ESCO models does DCI participate in?

Independence Kidney Care Alliance, LLC

Pittsburgh, PA | Liverpool, OH 

The Independence Kidney Care Alliance, LLC, serves a population of approximately 400 Medicare ESRD beneficiaries in Pennsylvania and Ohio.

Metropolitan Kidney Care Alliance, LLC

Newark, NJ | Staten Island, NY

The Metropolitan Kidney Care Alliance, LLC, serves a population of approximately 400 Medicare ESRD beneficiaries in Central New Jersey and Staten Island.

Music City Kidney Care Alliance, LLC

Nashville, TN

The Music City Kidney Care Alliance, LLC, serves a population of approximately 500 Medicare ESRD beneficiaries in Middle Tennessee. 

Palmetto Kidney Care Alliance, LLC

Spartanburg, SC

The Palmetto Kidney Care Alliance, LLC, serves a population of approximately 400 Medicare ESRD beneficiaries in South Carolina.
