Kidney Kamp Forms

We are excited to have you join us for camp! Let's get your information and get you set up. You will need to complete the forms below. Of course, campers need the Application, Emergency Medical Treatment, and Permission form. Counselors need the Counselor Application and the Counselor Contract. The forms can be filled out online, however, you must print and then mail or fax them in. The information on the screen will not be saved once you close the screen.
